When we schedule an appointment for you, we set that time aside, just for you, to address your oral health needs. Kindly provide us with 24 BUSINESS HOURS' notice if you need to change your appointment. Missing your appointment may deny another patient in need the use of our services and attention and may result in your dismissal from the practice. In addition, although we will make every effort to accommodate you and your family depending on the schedule that day, being 10 minutes late to your appointment may result in a reschedule. We strongly recommend you confirm your appointment ahead of time so as to ensure we are adequately staffed to meet your needs and can be seen in a timely fashion. Not doing so may result in a reschedule.

*****Please check our Facebook Page regularly  for updates regarding inclement weather or other emergencies that may impact your appointment.******

Your school or day care has forms they provide for your dentist to fill out for dental clearance. It is your responsibility to bring the required paperwork from said establishment for our office to fill out.

We are a GENERAL DENTIST office. We are extremely grateful and proud to be able to coordinate care for and ever-increasing amount of patients in Syracuse, NY and beyond. As a GENERAL DENTIST, Dr. Chobor is analogous to your Primary Care Physician. We welcome children, however we are NOT a PEDIATRIC DENTIST. Any and all work requiring SPECIALIST expertise to achieve optimum results in yours or your child's dental care will need to involve a SPECIALIST to meet those particular needs on a case by case basis - especially if there is a safety concern which we may not be equipped to handle. We are NOT licensed for SEDATION.

We participate with many insurances and are in the process of credentialing for more. It is your responsibility to know what your dental insurance plan covers and does not cover or if it is active. Our office provides these services as a courtesy. Predetermination of benefits for proposed treatment is ONLY an estimate and does not take into consideration any outstanding claims you may have incurred from other service providers that have not cleared yet or other exclusions. Any and all balances not covered by your insurance may become the patient's responsibility in accordance with the terms of your insurance company.

In order to establish baseline records for future comparison and proper diagnosis and treatment, Dr. Chobor does require comprehensive x-rays be taken at the first visit  (unless you are pregnant or think you may be pregnant.)

We use digital x-ray machinery in order to minimize the radiation exposure to our patients, as we know this is a major concern for some.  Cell phones, fluorescent lamps, microwaves and computers all emit small amounts of radiation. The radiation is a by-product of screens, the processes the allow products to cook, wireless signals and countless other things. These levels are so low that they pose no known risk to you or your health. You'll still receive more radiation from these sources than you will from x-rays at your dentist's office, which is true even if you see your dentist several times a year.

If you are pregnant or think you may be pregnant: We delay most treatment until after birth, unless it is an emergency. Please bring in clearance dated and signed by your OB-GYN provider prior to your first appointment in order to be seen right away for your emergency.

Patients under the age of 18 will only be seen with a parent or legally appointed guardian present. If you are getting dental work done, please have someone present to watch your underage child or children. No exceptions. This is a major safety concern for your children and cannot be negotiated.

Please feel free to click the link below to download our new patient paperwork. You can print it, fill it out, and bring it with you to your first appointment. You can use the form below to request an appointment. If you aren't able to fill the paperwork out before your appointment, it's highly recommended you arrive about 15 minutes early to allow ample time to complete any paperwork needed prior to your appointment.

Most Importantly: Thank you for considering our team for your dental needs!

Thank you for your interest in joining our practice!
